I was able to get an unlock code from T-Mobile, but no unlock
instructions were provided. Anyone knows what I need to do in order to
unlock my Motorola V195S?
Thanks.

As you already have the unlock code, you do NOT need any commercial
unlock service. Instead, simply put in any non T-Mobile SIM card and the
phone will ask you for the code. Enter the unlock code you have been
given and you are good to go.

Thank you, Andreas.
Just out of curiosity, is there a way of unlocking my cell phone while
the current SIM card (T-Mobile) is still in it? How?

I am not sure what good it would do so I am just curious to know if and
how it can be done; and if I unlock the phone now I don't have to worry
about the unlock code if and when I will install a non-T-Mobile SIM card...

Thanks.

This works on some models.

From the T-Mobile sunsidy password entry instructions for Motorola
phones:

If the display does not read "Enter Special code":
1. Press and hold the * key until an entry box is displayed, then
let go
2. Enter *, #, 3, 2, # and press "OK"
3. Enter the unlock code and press "OK." The display should read
"Completed" or "Deleted"
